Before You Begin
Please have available your social security card, names and dates of birth of all family members. Make sure you have correctly typed all information. Have paper and pen ready to write down your confirmation number.

Submit only ONE application to the City of Phoenix Housing Department (COPHD). Duplicate forms will not be accepted.

Maintaining Your Status on the Waiting List
Applicants must notify COPHD of any changes in address, phone, email or family members within 10 business days of the change. To update your information or to check the status of your pre-application, you may register and login using the MyHousing Applicant Portal:

The Applicant Portal requires registration and the use of the pre-application confirmation number. The Applicant Portal registration and pre-application status will be available approximately 24 hours after your pre-application submission.

You can also make changes to your pre-application by US Postal mail or in person at:

Section 8 Housing Choice Voucher: 830 East Jefferson Street, Phoenix, AZ 85034, or
Senior Housing Programs, Fillmore Gardens: 802 N 22nd Place, Phoenix, AZ 85006

You will be notified by mail when your name comes to the top of the waiting list or if we are updating our files. When you receive a letter from COPHD about your pre-application, you must respond by the date listed in the letter. If you do not respond to our notifications, or your letter is returned undeliverable, your name will be removed from the waiting list.

The Applicant Portal is your one-stop destination to:

  • Check the status of your pre-application.
  • Easily update any information that may have changed since you initially submitted your pre-application.
  • Apply for current open waitlists.
